Key Responsibilities\n

• Conduct comprehensive mental health assessments, taking into account each patient’s personal history, current issues, and risk factors, to develop suitable treatment plans.

• Implement evidence-based therapeutic techniques such as cognitive-behavioral therapy (CBT) and trauma-informed care to address a variety of psychological challenges faced by patients.
\n
• Facilitate individual therapy sessions that encourage emotional expressions, coping strategies, and personal growth, as well as lead group therapy sessions designed for shared experiences and mutual support.
\n
• Collaborate with an interdisciplinary team—including physicians and psychiatric professionals—ensuring an integrated approach to patient care and mental health support.
\n
• Engage in crisis intervention when necessary, helping patients and families navigate acute mental health distress while providing necessary support and resources.
\n
• Document and manage patient information using electronic health record (EHR) systems, ensuring accurate data that supports treatment and continuity of care.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, providing compassionate support during their end-of-life journey. You will engage with individuals and families, addressing not only mental health challenges, but also fostering coping mechanisms related to grief, loss, and the emotional complexities surrounding terminal illness. Your expertise ensures that patients feel supported in their mental and emotional well-being while receiving palliative care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active and valid LCSW licensure in New Jersey.
\n
• A minimum of two years of post-licensure experience working with diverse populations, preferably within a hospice or healthcare setting.

• Strong knowledge of mental health disorders, therapeutic modalities, and trauma-sensitive approaches.

• Experience conducting assessments, diagnosing mental health issues, and developing tailored treatment plans.

• Proficiency in facilitating counseling sessions for individuals and groups, employing various therapeutic techniques.

• Exceptional crisis intervention skills and the ability to remain calm and focused under pressure.

• Excellent interpersonal and communication skills, with a proven ability to build trusting relationships with patients and families from diverse backgrounds.

• Ability to work collaboratively with a multidisciplinary team focused on holistic patient care.

Preferred Experience\n

Compensation and Benefits\n
