Key Responsibilities\n

• Conduct thorough psychosocial assessments for individuals, adolescents, families, and groups, employing specified clinical social work/counseling interventions that align with client needs.

• Provide initial comprehensive patient and family psychosocial evaluations, accurately triaging care and developing individualized treatment plans.
\n
• Engage in collaborative discussions and documentation with multidisciplinary staff regarding assessments, treatment interventions, and patient care plans during staff meetings and patient rounds.

• Ensure a strong understanding of the physical, psychological, social, educational, and safety needs of patients, delivering competent care regardless of their age.

• Offer case management for assigned patients, engaging in consultations and collaborations with hospital and community professionals to facilitate cohesive and comprehensive care.

• Maintain ethical and timely completion of documentation and record-keeping as per departmental standards.
\n
• Implement Cognitive Behavioral Therapy (CBT) interventions along with other evidence-based treatment modalities to assist patients and families in fulfilling their individualized service plans.

Qualifications\n
