Terros Therapy Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team as a full-time remote specialist in hospice care. This position is fully remote, allowing for flexibility in work arrangements while still maintaining a strong connection with colleagues and clients. You will work collaboratively with a supportive team, utilizing telehealth tools and electronic health record systems to deliver high-quality care tailored to the unique needs of hospice patients and their families.

• Serve as a liaison with families and outpatient service providers to ensure coordinated and effective care that meets the needs of both patients and their families.
• Engage in thorough interactions with insurance companies for pre-certification and utilization management, ensuring that patients receive the appropriate services without unnecessary delays.
• Participate actively in discharge planning and aftercare strategies, ensuring smooth transitions and continuity of care for patients and their families.
• Provide therapeutic meal support and guidance, exemplifying ways to model a healthy relationship with food as part of overall wellness.
• Deliver safe, empathetic, and affirming care tailored to each patient’s unique circumstances and intersecting identities, recognizing the complexities that impact mental health.

This position primarily serves older adults receiving hospice care, addressing their mental health needs while also supporting their families. As an LCSW, you will be instrumental in recognizing and addressing the emotional challenges that arise in the hospice context, providing clients with resources, coping strategies, and supportive interventions during a vulnerable stage in life.

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of California is mandatory for this position.
• Demonstrated experience working with older adults, particularly in hospice or end-of-life care settings, is essential.
• Strong clinical and diagnostic skills are necessary to address a variety of mental health concerns.
• Excellent verbal and written communication skills are required for effective interaction with patients, families, and colleagues.
• The ability to work independently while also functioning as a proactive team member is critical for success in this role.

Experience with telehealth platforms and electronic health record (EHR) systems, as well as familiarity with hospice care protocols, will be an advantage. Additional training in grief counseling, palliative care, or related fields is also preferred.

We offer a competitive salary range of $90,000 to $105,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes:
