Key Responsibilities\n

• Perform comprehensive mental health assessments that take into consideration personal history, current emotional states, and social circumstances, ensuring a holistic understanding of the patient’s needs.

• Design and implement personalized treatment plans aimed at addressing significant mental health issues, thereby promoting emotional stability and coping strategies that help individuals navigate their hospice experience.

• Conduct evidence-based therapeutic interventions including cognitive-behavioral therapy (CBT) and trauma-focused therapy, tailored specifically to accommodate the unique challenges faced by patients in a hospice setting.

• Facilitate individual and family therapy sessions that encourage open communication about fears, grief, and adjustments related to end-of-life care.

• Offer crisis intervention during mental health emergencies, using de-escalation techniques, and providing referrals to community resources when necessary.

• Provide psychoeducation to patients and families regarding mental health conditions, treatment approaches, and available community and hospice support services.

• Maintain diligent and accurate documentation of patient assessments, treatment plans, and progress notes, adhering to all legal and ethical standards in mental health practice.

• Participate in regular team meetings and case conferences, contributing insights and strategies to improve service delivery and patient outcomes.

Patient Population / Client Focus\n

As an LCSW at our Hospice Center, you will primarily work with terminally ill patients and their families, providing essential psychosocial support during a challenging time in their lives. Your interactions will include addressing varied emotional responses to death and dying, navigating complex family dynamics, and offering bereavement support. Understanding the unique circumstances of each patient will be critical to fostering effective therapeutic relationships.

Qualifications\n

• A Bachelor's Degree in Social Work from an accredited institution is required.

• Must possess a current and valid LCSW licensure in the state of New Jersey.

• A minimum of two years post-licensure experience in a clinical mental health or hospice setting is required.

Preferred Experience\n

While not mandatory, experience in hospice or palliative care settings is strongly preferred, providing a deeper understanding of the specific emotional and psychological needs of patients facing end-of-life issues.

Compensation and Benefits\n

The salary range for this position is USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
