Work Environment

This position is performed onsite, where you will provide home care to our patients. Key Responsibilities

• Conduct thorough examinations and evaluations of patients presenting with musculoskeletal impairments, formulating evidence-based treatment plans tailored to individual needs.
• Provide comprehensive consultations to healthcare providers regarding patient care continuum, discussing treatment options and care pathways.
• Engage with patients of diverse backgrounds, ensuring respectful interaction in adherence with our customer service principles.
• Implement treatment plans that may include exercises, manual therapy, modalities, and patient education, continually re-evaluating patient progress and modifying treatment approaches as necessary.
• Document patient assessments, treatment outcomes, and discharge plans in compliance with regulatory standards while submitting timely charges for services rendered.
• Facilitate effective communication and collaboration within the interdisciplinary treatment team, ensuring seamless transitions of care and optimal recovery outcomes for patients.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients experiencing a range of conditions, such as post-operative recovery, sports injuries, and chronic pain syndromes. You will employ a hands-on approach to care, utilizing your clinical skills to support patients' physical rehabilitation in their own home environment, fostering a comfortable setting for recovery.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program is required.
• Active state licensure as a Doctor of Physical Therapist (DPT) in Florida.
• A minimum of two years of relevant clinical experience in orthopedic rehabilitation.
• Current Basic Life Support (BLS) or CPR certification to ensure patient safety in emergency situations.

Preferred Experience

• Experience with home health care delivery is preferred, as it allows for a more personal engagement with patients in their own environments.
• Familiarity with electronic health record systems for documentation and patient management.

Compensation and Benefits
