This position is uniquely structured to offer remote flexibility, allowing the LCSW to perform assessments and counseling from a home office while maintaining connection with clients and colleagues through virtual platforms. The role includes interaction with clients at our hospice center when necessary. The typical work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend availability needed to meet client demands and schedule constraints.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, carefully evaluating emotional, mental, and social factors influencing their well-being.

• Educate clients and family members on available community resources, social engagement programs, and support groups to encourage active participation in care and enhance quality of life.

• Maintain meticulous documentation of assessments, care plans, interactions, and progress notes in compliance with industry regulations and standards.

Patient Population / Client Focus\n

The primary focus of this role is on seniors receiving hospice care. This population may experience various challenges related to emotional and psychological aspects of end-of-life care. Hence, the LCSW will address issues related to grief, loss, and transitions, and assist clients and their families in navigating complex emotions during this difficult time.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, with a current LCSW license in Illinois.

• Minimum of two years of clinical experience, preferably in hospice, palliative care, or geriatric settings.
