Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients’ mental health, emotional state, and behavioral concerns.

• Develop and implement individualized care plans in collaboration with the treatment team, adjusting these plans based on ongoing assessments and patient feedback.

• Diagnose and treat a wide range of mental and emotional disorders while making necessary modifications to care plans to reflect changes in patient status.

• Facilitate connections between patients and various community resources, including support groups and specialized treatment programs, to enhance their overall well-being.

• Collaborate with a multidisciplinary team to create discharge plans that ensure continuity of care for patients transitioning from our services.

• Adhere to all relevant regulations and standards, acting within the scope of the Mental Health Practice Act.

• Perform social work consultation, including psychosocial assessments and supportive interventions in crisis situations.

• Provide education and communication to patients, families, and health care teams, ensuring a coordinated effort that supports patient care and addresses any delays.

• Advocate for patients and their families, facilitating effective dialogue among care teams, community resources, and agencies.

• Monitor patients for potential risks related to care transitions, assisting in optimizing discharge processes and minimizing the risk of readmission.

In this role, you will primarily work with diverse clients dealing with a range of mental health challenges, including anxiety, depression, trauma, and stress-related disorders. Qualifications\n

Compensation and Benefits\n

The annual salary for the Licensed Mental Health Counselor position ranges from USD 90,000 to 100,000, commensurate with experience. We also offer an enticing sign-on bonus up to $1,000.
