Key Responsibilities

• Conduct thorough evaluations of patients in a home care environment to assess their needs, limitations, and progress.
• Develop and implement individualized treatment plans using a variety of therapeutic methods, including exercise, manipulation, and modern rehabilitation equipment.
• Utilize evidence-based practices to help patients achieve their therapeutic goals, focusing on pain relief and improved mobility.
• Track and document patient progress through regular progress notes, evaluations, and adjustments to treatment plans as necessary.
• Educate patients and their families on exercises and methods to improve their health and independence.
• Advise on and fit patients for assistive devices, ensuring optimal support for daily activities.
• Stay current with industry trends and advancements by participating in continuing education, professional networking, and workshops.

Patient Population / Client Focus

Your work will focus on a broad range of patients, including children and adults recovering from injuries, surgeries, chronic illnesses, and neurological conditions. This diversity allows you to employ different therapeutic approaches, catering to individual circumstances and enhancing patient outcomes. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.

• Two years of relevant experience in a patient-facing role, particularly in home care, is ideal.
• Current CPR certification is required to ensure patient safety during treatment.
