Key Responsibilities

• Conduct comprehensive assessments to evaluate the mental health needs of clients, ensuring individualized treatment plans are developed and implemented.
• Engage in continuous communication with the treatment team, including supervisors, direct care staff, family members, and guardians to provide updates and insights on patient progress.
• Develop and maintain a structured schedule for client appointments, including telehealth sessions, home visits, and regular check-ins, to ensure adherence to treatment goals.
• Coordinate home visits where necessary to observe client behaviors, review charts, and conduct data analysis to enhance treatment strategies.
• Participate in Interdisciplinary Team Meetings to contribute insights and strategies regarding patients' care and treatment plans.
• Draft and submit monthly and quarterly progress notes to document client improvements and adjustments to treatment plans.
• Lead training sessions to educate staff on the effective implementation of treatment plans and documentation processes.
• Conduct follow-up interviews and home visits as necessary to provide ongoing support and evaluate the effectiveness of treatment interventions.

Patient Population / Client Focus

Qualifications

• A Master’s or Doctorate degree in a relevant mental health field is required.
• An active and unrestricted state licensure as one of the following: Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW).
• A minimum of two years of counseling experience, preferably in a clinical setting, is essential for this role.
• Strong understanding of legal and ethical standards within mental health practices is required.

Preferred Experience

• Specializations in family therapy, couples therapy, trauma recovery, or substance misuse treatment will be highly regarded.
• Evidence of successful collaboration within multidisciplinary healthcare teams.
