Position Overview

Century Impact Orthopedic Physical Therapy is actively seeking a dedicated and skilled Physical Therapist to join our team full-time based in Avenel, New Jersey. In this role, you will be pivotal in delivering personalized care to patients within their home environment, focusing on enhancing their mobility and quality of life. The Physical Therapist will primarily work within clients' homes throughout New Jersey, an ideal setting for applying clinical skills in a real-world context. Key Responsibilities

• Develop and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring functional mobility.
• Supervise and guide Physical Therapy Assistants in executing treatment plans effectively, ensuring compliance with best practices and state regulations.
• Conduct comprehensive evaluations of each client's physical condition, including mobility assessments and functional ability tests.
• Assess the home environment to recommend modifications that enhance the patient's independence and safety.
• Provide therapeutic exercises, manual therapy, and treatments involving physical agents such as traction and massage.
• Document clinical records meticulously, including evaluations, care plans, and progress notes, while maintaining compliance with regulatory standards.
• Collaborate with the interdisciplinary team during case conferences to ensure coordinated and comprehensive care.
• Engage with clients and their families, instructing them on rehabilitation techniques and the use of any therapeutic equipment.
• Monitor and report any changes in a client's condition, proposing necessary adjustments to the treatment plan as needed.
• Complete timely documentation of orders and other administrative tasks in accordance with company policies.

Patient Population / Client Focus

This position primarily involves working with a diverse patient population, including elderly clients, individuals recovering from surgery, and those with chronic illnesses or disabilities. Qualifications

• Graduation from an accredited Doctorate in Physical Therapy (DPT) program.
• Current PT licensure in good standing within the state of New Jersey.
• One year of experience as a Physical Therapist, preferably in home care or related settings.
• Current CPR certification.
• Strong interpersonal skills with a commitment to patient-centric care.
• Excellent decision-making abilities paired with a dedication to clinical excellence and integrity.
• Ability to work collaboratively and communicate effectively within a multidisciplinary team.

Preferred Experience

• Previous experience in a home health setting is preferred.

• Familiarity with Medicare and Medicaid regulations relevant to home healthcare.
• Experience in developing and leading patient education initiatives.

Compensation and Benefits

We offer a competitive yearly salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
