You will collaborate closely with a dedicated multidisciplinary team, including physicians, nursing staff, and support personnel, to coordinate care effectively. Key Responsibilities

• Conduct thorough initial evaluations to assess the patient's physical function and impairments, utilizing standardized diagnostic tests for muscle, nerve, joint, and functional abilities.
• Develop and implement tailored treatment plans aimed at reducing pain, restoring function, and improving the overall quality of life according to physician prescriptions.
• Oversee and mentor Physical Therapy Assistants in implementing treatment modalities, providing guidance and ensuring compliance with the plan of care.
• Administer a variety of therapeutic services, including manual exercises, massage, traction, and applications of physical agents.
• Analyze the home environment and recommend necessary modifications to enhance patient independence, safety, and rehabilitation effectiveness.
• Document patient progress meticulously, preparing clinical records including evaluations, care plans, and discharge summaries in accordance with regulatory requirements.
• Participate actively in interdisciplinary team meetings to assess treatment outcomes and adjust care plans in collaboration with healthcare professionals.
• Educate patients and families about home exercise programs, physical therapy devices, and the overall management of their treatment plans.
• Maintain current knowledge of regulations and best practices concerning home healthcare to ensure compliance and high standards of care.
• Respond to changing patient conditions promptly, adapting treatment strategies as needed, and communicate findings to appropriate stakeholders, including physicians and Case Managers.

Patient Population / Client Focus

This role primarily focuses on orthopedic patients requiring rehabilitation post-surgery, injury, or chronic pain management. You will assess and treat a diverse demographic, including geriatrics who often need specialized approaches to manage mobility and independence. Our approach emphasizes a holistic view of patient care, which means that you will not only focus on physical recovery but also consider emotional and cognitive well-being during therapy.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution; a Doctorate in Physical Therapy is preferred.
• Current state licensure (in good standing) as a Physical Therapist and CPR certification.
• Demonstrated ability to perform clinical assessments and construct effective treatment strategies.
• Strong interpersonal skills to create rapport with patients and collaborate effectively with healthcare professionals.
• Exceptional communication skills to instruct patients and provide clear guidance to their families.
• A commitment to ethical practice and professional development.

Preferred Experience

• Minimum of one year's experience as a Physical Therapist, preferably in an acute care or home care setting.
• Experience working with orthopedic patients and familiarity with rehabilitation technologies and methods is an advantage.
• A proven track record of effectively collaborating within an interdisciplinary team to enhance patient outcomes.

Compensation and Benefits
