Work Environment

This role involves providing onsite physical therapy services in the homes of our patients, allowing for a personalized care approach. Key Responsibilities

• Conduct thorough evaluations of patients, including detailed reviews of medical history and performance of specific physical assessments to form a comprehensive treatment plan.
• Communicate evaluation findings and treatment plans to patients and their families, engaging them actively in establishing individualized goals.
• Implement advanced therapy protocols tailored to each patient's unique circumstances, ensuring the best possible outcomes based on assessment data.
• Collaborate effectively with a multidisciplinary team, consulting as necessary and making timely adjustments to treatment plans based on documented changes in patient conditions.
• Handle discharge planning meticulously, ensuring that follow-up care is comprehensive and aligns with the established goals.
• Maintain accurate and timely documentation for all patient interactions, including initial evaluations, progress notes, and discharge summaries that reflect the patient's therapy journey.
• Participate in hospital and community marketing initiatives to promote physical therapy services and community health awareness.
• Take an active role in continuing education to ensure personal and organizational growth while mentoring future physical therapists.
• Promote a patient-centered care experience, demonstrating flexibility to adapt to changing patient needs and logistical circumstances.

Patient Population / Client Focus

Our primary focus is on individuals recovering from orthopedic conditions, including post-surgical patients, those experiencing sports-related injuries, and individuals with chronic musculoskeletal disorders. By offering therapy in the comfort of their homes, we facilitate a more comfortable and effective rehabilitation process, aligning with our goal of enhancing accessibility to care.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.
• A valid state license to practice as a Physical Therapist in Florida.
• Current CPR certification from the American Heart Association is required.
• Strong communication skills and the ability to work collaboratively within a team-based environment.

Preferred Experience

While prior work experience is not required, candidates with clinical placements or internships focusing on orthopedic rehabilitation will be viewed favorably. Exposure to home health care settings is a plus.

Compensation and Benefits
