Work Environment

This Physical Therapist position is primarily onsite within our home care facilities located in Fort Myers, Florida. Key Responsibilities

Your responsibilities as a Physical Therapist will include but are not limited to:

• Assessing patients through comprehensive evaluations to determine appropriate diagnoses and functional limitations.
• Developing individualized treatment plans that align with clinical guidelines and patient goals, ensuring a clear pathway to rehabilitation.
• Implementing therapeutic interventions utilizing proven protocols and adapting as necessary based on patient progress and health care developments.
• Continuously monitoring and documenting patient responses, and adjusting treatment plans in collaboration with other healthcare professionals when needed.
• Engaging in timely and accurate patient documentation, including initial evaluations, daily notes, interim progress summaries, and discharge documentation.
• Educating patients and their families about treatment methods, home exercises, and preventive care strategies that support ongoing recovery.
• Participating in facility meetings to enhance coordination of care and improve operational efficiencies.
• Staying current with industry trends and regulations, as well as completing any required training for compliance.

Patient Population / Client Focus

In this role, you will primarily work with patients experiencing orthopedic conditions, post-operative recovery, and chronic pain issues. Your expertise in developing and implementing rehabilitation strategies will be crucial for helping patients regain their independence and improve their overall quality of life. Our patient demographic is diverse, ranging from athletes to elderly individuals, all seeking effective rehabilitation from skilled professionals.

Qualifications

The following qualifications are required for this position:

• A Doctorate of Physical Therapy (DPT) from an accredited program.
• Active state licensure as a Physical Therapist from the Physical Therapy Examining Committee.
• A minimum of two years of experience working in an outpatient orthopedic setting.
• Current CPR certification is essential to ensuring patient safety and compliance with health regulations.

Preferred Experience

While not mandatory, candidates with additional certifications, such as Manual Therapy or Sports Rehabilitation, may have an advantage. Experience with electronic health record systems will also be beneficial in maintaining accurate documentation and enhancing patient care.
