Key Responsibilities

• Conduct thorough evaluations to assess patients' needs by reviewing historical information, performing specific assessments, and determining treatment recommendations and outcomes.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in setting achievable and meaningful goals.
• Implement effective treatment protocols tailored to each patient's unique presentation and the evolving healthcare landscape, ensuring that care is evidence-based.
• Collaborate with other healthcare professionals when necessary and diligently document any changes in the patient's condition that could affect their treatment plan.
• Manage discharge planning comprehensively, ensuring continuity of care by providing clear documentation of each patient's progress, which includes daily encounter notes and interim summaries.
• Demonstrate excellent time management skills, maintaining an accurate patient schedule and meeting productivity expectations, which drives our success.
• Take proactive steps in participating in community outreach and hospital marketing activities to promote physical therapy services.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
