Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and capabilities, analyzing their medical history and current functional status to develop a comprehensive treatment plan.
• Implement and monitor treatment interventions while assessing patient responses to ensure the effectiveness of the proposed therapies.
• Document all patient care activities accurately, following state and federal regulations as well as company policies.
• Educate and guide patients and their families, equipping them with the knowledge and tools necessary to manage their conditions effectively, both during therapy and beyond.
• Coordinate treatment plans with interdisciplinary teams, ensuring a unified approach to patient care that addresses all aspects of individual patient needs.
