Work Environment

This role requires an onsite presence as our Physical Therapists visit clients in various home settings throughout North Fort Myers and its surrounding areas. Key Responsibilities

You will be primarily responsible for implementing and monitoring personalized treatment plans to achieve optimal patient outcomes. Specific responsibilities include:

• Directing physical therapy treatment aimed at preventing disability, alleviating pain, and enhancing physical function based on each patient's unique needs and physician prescriptions.
• Conducting thorough evaluations of patients' functional abilities through diagnostic tests and assessments, determining the most effective rehabilitation strategies.
• Oversight of Physical Therapy Assistants to ensure the delivery of quality care according to established treatment plans.
• Evaluating patient home environments to identify barriers to rehabilitation and recommending modifications to promote independence.
• Administering a variety of therapeutic techniques, including manual exercises, traction, massage, and the application of physical agents.
• Fitting and adjusting prosthetic and orthotic devices as required, providing patients with guidance on their proper use.
• Regularly observing and recording patient progress, reporting changes to the treatment plan to physicians and case managers.
• Maintaining comprehensive clinical documentation including initial evaluations, progress notes, and discharge summaries, adhering to OASIS guidelines.
• Engaging actively in interdisciplinary team discussions and care conferences to promote comprehensive treatment strategies.
• Conducting patient and family education to optimize the use of therapy devices and increase understanding of the rehabilitation process.

Qualifications

To be considered for this position, you must possess:

• A Bachelor's Degree in Physical Therapy from an accredited program; a Doctorate in Physical Therapy is preferred.

• A valid driver's license and automobile insurance, showcasing reliability to travel to patient homes.

• At least one year of experience in physical therapy within an acute care or home care setting.

Compensation and Benefits
