Work Environment

This role primarily involves onsite home care service, allowing you to directly engage with patients in their own living environments. You'll be part of a collaborative clinical team, working in varied home settings that can range from cozy apartments to larger family residences. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, developing individualized treatment plans that respect their specific conditions and capabilities.
• Implement hands-on therapy techniques and therapeutic exercises tailored to enhance recovery, mobility, and overall quality of life.
• Monitor and document patient progress meticulously, adjusting treatment plans as necessary in collaboration with the healthcare team.
• Educate patients and caregivers about exercise regimens, injury prevention, and lifestyle adjustments that can support long-term wellness.
• Develop strong relationships with each patient, ensuring they feel supported and understood throughout their treatment journey.
• Meet established performance metrics and provide exceptional customer service to enhance the patient experience.

Patient Population / Client Focus

Our patient demographic includes individuals of all ages with diverse conditions, including but not limited to orthopedic, pediatric, neurologic, and sports medicine issues. As a home care specialist, your ability to adapt your treatment approach to the specific atmosphere and needs of each patient will be crucial in fostering recovery. You will engage with patients one-on-one, addressing diverse needs from basic mobility enhancement to advanced rehabilitative practices.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program recognized by APTA or CAPTE.
• Active physical therapy license in the state of Florida, ensuring compliance with state regulations.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Excellent written and verbal communication skills to interact effectively with patients and their families.
• Proficiency in computer skills, including record-keeping and using electronic health record systems.

Preferred Experience

We are seeking candidates with a minimum of two years of direct experience in physical therapy, particularly in home care settings. Experience in a variety of specialties, including orthopedic rehabilitation, pediatric therapy, and neurologic rehabilitation, is highly valued. A strong background in evidence-based practice will enhance your ability to provide optimal patient care.

Compensation and Benefits

At Oakbend Physical Therapy, we offer a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience, along with a comprehensive benefits package.
