Work Environment

As an onsite Physical Therapist working within the home care framework, you will engage directly with patients in their homes, providing personalized care tailored to their unique needs. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing specific assessments, and developing tailored treatment plans that address individual needs.
• Communicate evaluation results and treatment recommendations clearly to patients and their families, involving them in setting achievable health goals.
• Utilize evidence-based treatment protocols that align with the latest advancements in healthcare, adapting strategies based on ongoing patient assessments and progress.
• Collaborate with interdisciplinary teams to enhance patient care and identify necessary adjustments to treatment plans based on shifts in patient conditions.
• Document all patient interactions accurately and in a timely manner, including evaluations, treatment notes, progress updates, and discharge summaries, ensuring compliance with healthcare regulations.
• Demonstrate effective time management by maintaining an organized schedule and meeting productivity expectations, while ensuring punctuality for meetings and patient visits.
• Participate actively in departmental initiatives to remove barriers to patient access, leveraging metrics from the site's CSI measurement data to improve service delivery.
• Engage in community-based marketing activities to promote the organization's services and foster relationships that enhance our outreach and support services.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including those recovering from surgery, individuals with chronic pain, and clients with mobility challenges related to various conditions. Our team is committed to providing tailored care strategies that are culturally competent and sensitive to the individual needs of our patients.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid license to practice as a Physical Therapist in Florida, or eligibility to obtain one.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• While prior experience in physical therapy is appreciated, it is not a requirement. New graduates are encouraged to apply.

Preferred Experience

Experience in home health care or outpatient physical therapy settings is advantageous but not mandatory. Familiarity with electronic health records (EHR) systems and a commitment to maintaining ongoing professional development through continuing education are also encouraged.

Compensation and Benefits
