Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to identify the mental health, substance abuse, and medical needs of patients, ensuring culturally appropriate evaluations.

• Provide therapeutic services that include individual and group counseling, intake assessments, treatment planning, crisis intervention, and support for patients dealing with end-of-life issues.

• Collaborate with physicians and healthcare teams to escalate patient care needs, ensuring timely interventions that enhance overall well-being.

• Maintain accurate and thorough electronic documentation of progress notes and assessments in accordance with organizational protocols and regulations.

• Facilitate access to community resources and support organizations, including medication assistance programs, fostering a network of care for patients and their families.

• Work with quality improvement teams to develop and implement processes that enhance data collection methodologies and improve service delivery.

Qualifications\n

• A current and active state license as a Licensed Clinical Social Worker (LCSW) is required.
