Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through detailed patient interviews, observations, and medical record reviews to evaluate the factors impacting student recovery.

• Provide ongoing counseling and emotional support to students and their families, facilitating their adjustment to challenges arising from disabilities or health conditions.

• Collaborate with the rehabilitation team to design and implement individualized treatment plans that address the psychosocial needs of each student.

• Help students and families navigate community resources, support services, and educational programs aimed at enhancing post-rehabilitation success.

• Educate students and their families about available community services and assist in the transition between rehabilitation and school settings.

• Participate actively in discharge planning, ensuring a smooth transition for students returning to their educational journeys.

Patient Population / Client Focus\n

In this role, you will primarily work with a diverse population of school-aged students recovering from various conditions, including physical injuries, neurological disorders, and emotional challenges. Your expertise will assist these young individuals in overcoming barriers to their education and social functioning, ultimately enabling them to thrive and succeed academically and personally.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active California state licensure as a Licensed Clinical Social Worker (LCSW).
