Key Responsibilities\n

• Conduct thorough and ongoing assessments of patients to accurately understand their mental health needs and to inform treatment planning.

• Facilitate open and effective communication among supervisors, direct care staff, family members, and other relevant parties regarding a patient’s progress and treatment adjustments.

• Establish a tentative schedule for patient appointments, home visits, and observations, maintaining flexibility to adapt as needed based on client circumstances.
\n
• Coordinate and execute home visits as required, which includes performing in-home observations and chart reviews to support holistic treatment approaches.
\n
• Ensure effective implementation of personalized treatment plans, modifying them as necessary based on ongoing evaluations.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights and updates that help shape comprehensive care strategies.
\n
• Maintain detailed monthly and quarterly progress notes, documenting treatment efficacy and patient engagement.
\n
• Design and deliver training sessions for team members, ensuring they are well-prepared to implement program plans and meet documentation standards.
\n
• Conduct follow-up interviews and additional home visits to provide continued support for clients, reinforcing their treatment journey.
\n
• Maintain accurate records to ensure information is readily accessible to authorized individuals involved in patient care.
\n
• Serve as a liaison with the larger mental health community, contributing to initiatives that support patient development goals and enhance community relations.

• Contribute to the development of training materials and procedures aimed at enhancing program delivery and effectiveness for our clients.

Patient Population / Client Focus\n

Our practice serves a varied population, including both adults and youth. You will work closely with clients dealing with complex mental health challenges, including but not limited to family dynamics, substance misuse, and trauma recovery. Your expertise in these areas will be critical in building trusting therapeutic relationships that lead to successful outcomes.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Licensed Professional Counselor in the state of Pennsylvania.

• Minimum of 2 years of clinical counseling experience, ideally within an outpatient or community mental health setting.

• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma work is highly desirable.

• Solid understanding of legal and ethical standards governing counseling practices.

• Strong interpersonal and communication skills, effectively establishing rapport with a diverse range of clients.

• Demonstrated ability to work collaboratively within a multidisciplinary healthcare team.
