Key Responsibilities\n

As a Licensed Professional Counselor at Mountain Springs, your responsibilities will include:

• Supporting primary care providers by recognizing, treating, and managing patients' mental health and psychosocial issues.

• Conducting patient intakes focusing on detailed diagnostic and functional evaluations to recommend personalized treatment goals.
\n
• Providing brief, focused interventions for patients seeking mental health services and guiding them through therapeutic processes.
\n
• Advising primary care teams on which patients may benefit from specialty mental health services versus those that can continue care in primary settings.

• Initiating follow-up evaluations to monitor patient progress and adjusting treatment plans as necessary.
\n
• Developing relapse prevention strategies to help patients maintain stable functioning and detect at-risk individuals.
\n
• Documenting patients' progress and diagnostic information accurately within treatment charts, ensuring compliance with agency and Federal Qualified Health Center (FQHC) guidelines.

• Educating patients on self-management skills and coping strategies to actively participate in their recovery.
\n
• Collaborating with clinic management to enhance behavioral health services and contribute to program evaluations.

• Assisting patients in complying with medical treatments and supporting them in managing side effects.
