Key Responsibilities

• Conduct comprehensive physical therapy evaluations for patients to identify needs, develop treatment goals, and design individualized therapy plans.
• Implement and modify treatment interventions based on patient progress, clinical findings, and specific recovery goals.
• Maintain meticulous documentation of patient care, including SOAP notes, progress reports, and assessments through our electronic health record (EHR) system.
• Educate patients and their families about treatment plans, recovery processes, and management of conditions, ensuring they are engaged and informed throughout their therapy journey.
• Collaborate with an interdisciplinary team to provide coordinated care for patients receiving multiple therapy services.
• Achieve and sustain minimum utilization goals and contribute to overall business objectives of Heal Motion Care.
• Serve as a culture champion for Heal Motion Care, embodying our core values and fostering a positive team environment.

Patient Population / Client Focus

The ideal candidate will be enthusiastic about working with a diverse patient population, including children and adults from various backgrounds and health conditions. Our clients may present with acute and chronic injuries, developmental delays, neurological conditions, and post-operative rehabilitation needs. By utilizing a patient-centered approach, you will adapt your treatment strategies to fit the unique circumstances and goals of each individual.

Qualifications

• A Master's or Doctorate in Physical Therapy from an accredited institution is required.
• Current and unrestricted licensure as a Physical Therapist in the state of California.
• Capable of conducting in-person evaluations and treatments, with a willingness to embrace telehealth when clinically appropriate.

Compensation and Benefits
