Work Environment\n

This remote position allows you the flexibility to manage your work-life balance while collaborating closely with our integrated care team that includes psychiatrists, psychologists, and other healthcare professionals. Your primary working hours will be from Monday to Friday, 8:00 AM to 5:00 PM, with occasional evening shifts as patient needs dictate. Key Responsibilities\n

• Conduct in-depth mental health assessments, evaluating the interplay between patients’ mental health and their medical conditions.

• Develop and execute personalized, integrated treatment plans that address both psychiatric and physical health issues.

• Lead individual and group therapy sessions utilizing evidence-based therapeutic approaches such as Cognitive Behavioral Therapy (CBT) and other relevant modalities.

• Collaborate effectively with medical providers to ensure a unified approach to care, enhancing treatment effectiveness and patient outcomes.

• Provide crisis intervention and immediate support to patients experiencing mental health emergencies.

• Educate patients about the importance of mental health in relation to their physical health, empowering them to actively participate in their treatment journey.

• Maintain meticulous case documentation according to regulatory standards to ensure compliance and confidentiality.

• Engage in regular team meetings and case discussions to promote interdisciplinary collaboration and ongoing care planning.

Patient Population / Client Focus\n

You will primarily be working with individuals who face complex medical conditions in conjunction with mental health challenges. This includes patients experiencing chronic illnesses, substance use disorders, and significant life stressors. Your role is vital in bridging the gap between mental health and physical health, ensuring a comprehensive approach to patient care.

Qualifications\n

• A master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LPC, or LMFT in the state of Arizona.

• Demonstrated proficiency in working with patients who have co-occurring disorders, integrating mental health with physical health considerations.

• Prior experience in a collaborative integrated care setting is strongly preferred.

• Excellent interpersonal skills, with the ability to foster trusting relationships with patients and colleagues alike.

Compensation and Benefits\n
