This is a remote position, allowing for flexible work hours to help you maintain an optimal work-life balance. While working from home, you will have the opportunity to connect with clients primarily through telehealth services, supplemented by necessary in-person visits when feasible. Key Responsibilities\n

• Conduct comprehensive assessments to tailor counseling approaches that meet the specific needs of each patient.

• Maintain effective communication with supervisors, direct care staff, family members, and other stakeholders regarding patient progress, ensuring a collaborative treatment approach.

• Develop a schedule for appointments, home visits, and monitoring of client progress, guaranteeing adherence to treatment plans.

• Conduct home visits when necessary, utilizing these opportunities for in-depth observations, chart reviews, and interviews to obtain a holistic view of the patient's situation.

• Implement and oversee the effectiveness of treatment plans, making adjustments as needed to ensure optimal patient outcomes.

• Participate actively in Interdisciplinary Team Meetings and other relevant gatherings to discuss patient care strategies.

• Document monthly and quarterly progress notes, providing clear documentation of therapeutic progress and challenges.
\n
• Facilitate training sessions for staff to ensure proficiency in the execution of program plans and best practices for documentation.
\n
• Provide ongoing support through follow-up interviews and observations to reinforce each individual's progress and needs.
\n
• Maintain thorough records accessible to relevant parties, ensuring transparency and compliance with mental health standards.

• Serve as a liaison with the wider mental health community, contributing to the development and improvement of service delivery.

• Assist in crafting training materials to enhance program effectiveness for clients.

Patient Population / Client Focus\n

As an LPC at Motion Rehab Group, you will work with a diverse clientele, including both adults and youth. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
