Key Responsibilities

• Conduct thorough psychosocial assessments using structured interviews, patient observations, and comprehensive reviews of medical records to gauge patients’ emotional, social, and environmental challenges that may be hindering their rehabilitation.
• Provide individual and group counseling for patients and their families, addressing significant adjustment issues and promoting emotional resilience throughout the rehabilitation process.
• Collaborate actively with the multidimensional rehabilitation team to devise and implement tailored treatment plans that holistically address the psychosocial components vital for patient recovery.
• Assist patients in accessing vital community resources, such as local support groups, vocational rehabilitation programs, and assistance for housing or transportation, ensuring they have the comprehensive support needed during and after rehabilitation.
• Educate patients and their families on available services and resources, enhancing their understanding of post-rehabilitation support and facilitating smoother transitions to home care or further treatment settings.
• Participate in collaborative discharge planning, helping patients successfully transition from our rehabilitation facility back into their communities.

Patient Population / Client Focus

Your work will directly impact a diverse population, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. By addressing both psychological and emotional needs, you will help patients regain control over their lives and promote their overall well-being.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• An active license to practice as a Licensed Clinical Social Worker (LCSW) in Texas.
• Demonstrable knowledge and experience in mental health care, specifically addressing adjustment issues, grief, and coping strategies.
• Exceptional communication and interpersonal skills, allowing you to build rapport with patients, families, and colleagues.

Preferred Experience
