Key Responsibilities

• Conduct thorough psychosocial assessments through patient interviews, clinical observations, and careful review of medical records to understand emotional, social, and environmental factors influencing recovery.
• Provide individual and family counseling to assist patients in coping with the emotional challenges of their conditions, addressing adjustment issues, and promoting mental well-being.
• Collaborate with a multidisciplinary rehabilitation team to design and implement individualized treatment plans that integrate patients’ psychosocial needs.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring seamless access to essential services.
• Educate patients and their families about available community resources and offer guidance on post-rehabilitation options to facilitate a successful transition back to everyday life.
• Engage actively in discharge planning, coordinating the necessary steps to ensure patient safety and continuity of care following rehabilitation.

Patient Population / Client Focus

In this role, you will primarily support a diverse patient population consisting of individuals recovering from various conditions, including strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Each patient presents unique emotional and practical challenges; your expertise in social work will be key in addressing these needs and guiding each individual toward achieving their highest level of independence.

Qualifications

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication skills, allowing for effective interaction with patients, families, and interdisciplinary team members.

Preferred Experience

• Previous experience in a rehabilitation or medical setting is highly desirable.

Compensation and Benefits
