Key Responsibilities\n

• Conduct in-depth psychosocial assessments of senior clients to determine their unique needs and preferences, taking into account emotional, mental, and social factors.

• Develop personalized care plans in collaboration with clients and their families, aiming to address specific goals related to health, well-being, and independence.

• Provide individual counseling sessions, offering emotional support to seniors facing life transitions, health challenges, or family dynamics that may affect their well-being.

• Advocate for clients' rights and help them navigate access to essential community resources and services, ensuring they receive appropriate support.

• Engage in regular virtual visits to monitor clients' progress, assess living conditions, and identify any areas that may require additional support or intervention.

• Facilitate communication among family members and involved parties to resolve conflicts and promote collaborative care for the client.

• Educate clients and their families about local resources, support groups, and programs that can enhance social engagement and improve quality of life.

• Maintain accurate electronic health records (EHR) by documenting client assessments, care plans, progress notes, and relevant interactions, ensuring compliance with ethical and regulatory standards.

This role primarily focuses on serving seniors within the Washoe Valley area who may be experiencing various challenges related to aging, including isolation, health declines, and family dynamics. Qualifications\n

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Nevada.

• A Bachelor’s degree in Social Work from an accredited institution; a Master’s degree is preferred.

• A minimum of two years of relevant social work experience, particularly in geriatric or community health settings.

• Strong understanding of social work principles, aging-related issues, and local community resources.

• Excellent assessment, counseling, and problem-solving abilities.

Preferred Experience\n

Compensation and Benefits\n

We offer a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
