Key Responsibilities\n

• Conduct comprehensive clinical assessments to identify the psychological, social, and emotional needs of patients.

• Provide evidence-based short-term psychotherapy and group therapy to patients, utilizing various therapeutic modalities relevant to their conditions and preferences.

• Develop, implement, and evaluate individualized treatment plans based on the clinical assessments and ongoing patient feedback.

• Facilitate outreach activities, collaborating with other healthcare professionals and community organizations to ensure comprehensive care.

• Deliver crisis assessment and intervention services, employing structured methodologies to ensure patient safety when immediate assistance is needed.

• Engage in continuous professional development through training and workshops to enhance skills and stay updated with the latest therapeutic practices.

• Respond to situations involving suicidal ideations, coordinating hospital admissions when necessary and formulating appropriate safety plans in collaboration with the patient and their support networks.

• Document all clinical interactions accurately and in compliance with legal and ethical standards, ensuring thorough communication within the healthcare team.

• Provide consultation on complex cases, guiding other staff members in addressing counseling needs.

Patient Population / Client Focus\n

Your primary focus will be on a diverse population, including adolescents and college students who are navigating mental health challenges, particularly in a hospice context. Qualifications\n

• A Master’s Degree in Social Work (MSW) is required, along with a valid Licensed Independent Clinical Social Worker (LICSW) credential. Candidates with a fully licensed LMHC designation will be considered.

• A minimum of three years of post-master’s clinical experience in social work, particularly in case management policies and practices.

• Proficiency in delivering treatment to a diverse adolescent or college-aged population, demonstrating an understanding of the unique challenges faced by these groups.
