Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for clients upon admission, annually, and as clinically indicated to identify and address barriers to care.

• Collaborate with interdisciplinary teams to create, implement, and monitor psychosocial treatment plans that integrate various therapeutic approaches to meet the unique needs of each patient.

• Facilitate supportive and therapeutic counseling sessions, providing crisis intervention and grief counseling to assist clients in navigating life's challenges.

• Manage a diverse caseload independently, ensuring the delivery of effective mental health services tailored to varying levels of clinical acuity.

• Participate actively in team conferences and interdisciplinary rounds, identifying patients and families with potential psychosocial needs for further assessment.

• Document patient progress meticulously through electronic medical records, adhering to organizational policies to ensure continuity of care.

• Engage in community improvement initiatives and contribute to the development of social work service guidelines that align with organizational goals.

Patient Population / Client Focus\n

Your work will impact a wide-ranging clientele, including adults, the elderly, children, and adolescents dealing with diverse mental health challenges. Our client base may present with issues related to anxiety, depression, trauma, chronic illness, and other psychological conditions. You will be responsible for helping them cope with both environmental and psychological aspects of their conditions, guiding them towards resilience and improved mental health.

Qualifications\n

• A Master's degree in a behavioral science, social work, or related field from an accredited institution is required.

• Possession of an active and valid mental health licensure (LCSW, LMFT, or LPCC) in California.

• Strong abilities in written communication, particularly when documenting case narratives, treatment plans, and patient progress reports.

• Proficient in applying effective assessment techniques and counseling methodologies in clinical practice.

• Ability to build rapport with clients and facilitate therapeutic engagement in individual and group settings.

Preferred Experience\n

Compensation and Benefits\n
