Key Responsibilities

• Conduct thorough evaluations of patients, reviewing medical histories and performing specific assessments to determine their individual needs and craft tailored treatment plans.
• Clearly communicate evaluation findings and treatment options to patients and their families, involving them in establishing realistic goals for recovery.
• Demonstrate expertise in implementing customized treatment protocols that reflect the latest evidence-based practices and the patient's unique circumstances.
• Consult with fellow healthcare professionals as necessary, carefully monitoring and documenting changes in patients' conditions and modifying care plans accordingly.
• Oversee the discharge planning process, ensuring continuity of care, appropriate follow-up, and comprehensive documentation of services provided.
• Maintain effective time management skills to meet productivity targets while ensuring high-quality patient care, including punctual attendance at meetings and adherence to schedule.
• Contribute to departmental objectives and compliance with all regulatory and accreditation standards.

Patient Population / Client Focus

Our patients range from those recovering from surgery to individuals seeking rehabilitation from chronic pain or mobility issues. You will work closely with diverse populations, adapting treatment methods to accommodate varying needs, abilities, and rehabilitation goals.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current and valid licensure to practice Physical Therapy in Virginia is mandatory.
• Basic Life Support (BLS) certification from the American Heart Association for Healthcare Providers.

Preferred Experience

• While a minimum of 2 years of experience as a Physical Therapist is preferred, we are open to considering passionate candidates transitioning into the home care environment or new graduates with relevant clinical experience.
