Heritage Wellness Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. This remote position focuses on providing essential outpatient therapy to individuals and families navigating the challenges brought on by cancer diagnoses. Our ideal candidate will possess a Master’s degree in Social Work and have an active LCSW license in New Jersey. Key Responsibilities

• Conduct thorough assessments of patients to identify their mental health status and any progress toward therapeutic goals, utilizing tools consistent with evidence-based practices, such as the Diagnostic and Statistical Manual of Mental Disorders.

• Develop and implement individualized treatment plans that encompass culturally responsive strategies and address the unique needs of each patient.

• Continuously monitor and evaluate patient progress and adjust treatment strategies as necessary to ensure optimal outcomes.

• Prepare and maintain accurate and timely treatment records in compliance with regulatory requirements, using current electronic health record (EHR) technology.

• Work collaboratively with patients, their families, and a multidisciplinary team to create comprehensive care plans that prioritize safety and symptom management.

• Act as a liaison between patients and external treatment providers, coordinating referrals and establishing successful working relationships with community resources.

• Engage in educational initiatives by mentoring interns, students, and volunteers, contributing to the development of future healthcare professionals.

• Participate in peer review and collaborative clinical activities, supporting ongoing development and improvement of the team.

Patient Population / Client Focus

The Licensed Clinical Social Worker will primarily focus on individuals and families affected by cancer. This client population may experience heightened anxiety, emotional distress, and psychosocial challenges as they navigate their diagnoses and treatment options. Providing compassionate and empathetic care is crucial, as many patients may be facing significant life transitions and stressors.

Qualifications

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.

• Strong understanding of mental health diagnoses and treatment modalities.

Preferred Experience

• Experience working within oncology or with patients undergoing significant health issues.

• Proficiency in utilizing electronic health record (EHR) systems for documentation.

• Previous involvement in community outreach or engagement initiatives.
