Key Responsibilities\n

As a Licensed Mental Health Counselor, your key responsibilities will include:

• Communicating effectively with physicians and healthcare providers to report on patient progress, needs, and any concerns or complications that may arise.

• Maintaining accurate and timely clinical documentation, including progress notes and treatment plans, in accordance with departmental policies and standards.

• Collaborating with the healthcare team to develop and implement individualized treatment plans, including tailored discharge plans based on psychosocial assessments.

• Counseling clients and providing crisis intervention to address emotional and behavioral challenges, while employing evidence-based therapeutic methodologies.

• Facilitating client discharge planning and conducting care coordination efforts to ensure a seamless transition back into the community, working closely with regional healthcare providers.

• Consulting and coordinating with families and support systems to enhance treatment efficacy and discharge processes.

Qualifications\n

The ideal candidate for this position will meet the following qualifications:

• A Master’s degree in Social Work, Counseling, or Marriage and Family Therapy is mandatory.

• A minimum of 2 years of clinical experience in an outpatient setting, providing counseling and therapeutic interventions.

• Strong computer literacy, including proficiency in word processing, email, spreadsheets, and electronic medical records management.

• The ability to work independently and remain self-directed, demonstrating high levels of organization.

• Proficiency in MS Office Suite and experience with virtual telehealth platforms.

Compensation and Benefits\n
