Key Responsibilities\n

• Provide expert clinical consultation to clinical staff regarding complex mental health issues, working with various patient populations, including geriatric and individuals struggling with substance use disorders.

• Communicate critical information concerning patients and their families, particularly social histories, to enhance coordination among staff and healthcare providers.

• Coordinate interdisciplinary meetings to develop and implement tailored patient care plans, emphasizing engagement with the patient's home circumstances and support systems.

• Design and review effective treatment plans and provider cases, ensuring alignment with each client’s therapeutic goals.
\n
• Conduct thorough progress notes following client sessions to document treatment effectiveness and inform required adjustments in care.

• Engage in ongoing professional development and attend team meetings to share insights, present case studies, and discuss best practices.

Patient Population / Client Focus\n

As a member of our team, you will work with a diverse client base, addressing the varying needs of individuals from different cultural backgrounds. Qualifications\n

• A Master's Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Must possess at least one of the following independent licenses: LMHC, LCSW, or LMFT.

• Strong proficiency in completing progress notes efficiently while adhering to best practices and regulatory standards.

• Exceptional communication skills and the ability to build rapport with clients in a virtual setting.

Preferred Experience\n

• Experience working with elderly clients or individuals with substance use disorders is highly desirable.

• Familiarity with telehealth platforms and electronic health record (EHR) systems will be an advantage.

Compensation and Benefits\n
