As an integral member of our hospice center team, you will collaborate with a multidisciplinary group of healthcare professionals, including physicians, nurses, and rehabilitation specialists, all committed to delivering comprehensive care. We utilize state-of-the-art electronic health record systems, allowing for seamless communication and documentation among team members.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients’ emotional, social, and environmental needs, ensuring personalized care plans are developed and implemented.

• Facilitate therapy sessions using evidence-based methods tailored to hospice patients, focusing on grief, loss, and coping strategies.

• Communicate effectively with patients and their families, providing education and support during challenging times and ensuring they are informed about available resources.

• Collaborate with healthcare professionals across the organization to contribute to interdisciplinary care meetings and ensure coordinated care strategies.

• Participate in staff training programs to enhance the knowledge and skills of new employees as required.

• May be requested to cover additional facilities and participate in outreach programs to expand care access.

• Document all interactions and updates in our electronic health record system to maintain compliance with organizational and regulatory standards.

Patient Population / Client Focus\n

This role primarily focuses on patients receiving hospice care, which often includes individuals facing terminal illnesses. Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

• Active and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania is essential.

Compensation and Benefits\n
