Key Responsibilities\n

• Actively participate in patient transition conferences, facilitating communication between clients, families, and healthcare teams.

• Stay informed about community resources, government programs, and private payer systems to assist clients in their care transitions.

• Perform other duties as assigned, maintaining adaptability in a fast-paced and evolving environment.

Patient Population / Client Focus\n

Your primary focus will be on clients grappling with a range of mental health issues, including anxiety, depression, trauma, and the emotional impacts of severe illnesses. The therapist will also provide support to family caregivers, offering educational resources and counseling to help them manage their unique challenges while fostering the overall well-being of their loved ones.

Qualifications\n

• A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

• Must hold an independent license in the state of Oregon where you reside (e.g., LCSW, LMFT, LPC).

• Proficient in electronic correspondence and skilled in utilizing EMR systems for patient documentation and communication.

Compensation and Benefits\n
