Lead Physical Therapist - Outpatient Orthopedic Practice in Assisted Living

St. Elara Medical Center is seeking a motivated and empathetic Physical Therapist to join our outpatient orthopedic practice. In this full-time position, you will provide essential physical therapy services on-site at various assisted living facilities (ALFs) in East Rutherford, New Jersey. Work Environment

The physical therapist will work onsite within assisted living facilities, offering a distinctive blend of clinical practice and personal interaction with residents. Working hours are Monday to Friday, 9 AM to 5 PM, providing a structured schedule that allows for work-life balance and ample time to focus on patient needs.

Key Responsibilities

• Deliver tailored on-site physical therapy services to residents in affiliated assisted living communities, ensuring that treatments are suitable for their specific needs and conditions.
• Conduct thorough evaluations and assessments to create customized, orthopedic-informed treatment plans that address both rehabilitation and functional performance.
• Collaborate effectively with facility nursing staff and caregivers to ensure cohesive care, reinforcing communication between the therapists and clinical team.
• Educate residents and their families on safe mobility techniques, develop home exercise programs, and guide them in wellness strategies to support ongoing health.
• Utilize mobile electronic medical records (EMR) systems for efficient and compliant documentation of patient interactions and treatment plans, keeping accurate and up-to-date records.
• Act as an essential liaison between the assisted living facility and our multidisciplinary clinic to facilitate referrals and follow-ups for comprehensive care.

Patient Population / Client Focus

The primary focus of this role is the aging adult population residing in assisted living facilities, who may present with a variety of orthopedic conditions commonly found in older adults, such as arthritis, post-operative rehabilitation needs, and mobility impairments. Your interventions will be instrumental in enhancing their daily living and mobility, resulting in improved overall well-being.

Qualifications

• Must possess a valid Physical Therapist license in the State of New Jersey or be eligible for licensure.
• A Bachelor's Degree in Physical Therapy or a related field is required.
• Excellent communication and interpersonal skills to foster relationships with patients, families, and interdisciplinary staff.
• Understanding of orthopedic conditions prevalent in elderly populations and the ability to tailor approaches accordingly.

Preferred Experience

• Minimum of two years of hands-on experience in outpatient physical therapy environments, preferably with a specialization in geriatrics, assisted living, skilled nursing facilities (SNFs), or home care settings.
• Proficient in utilizing electronic medical records (EMR) and maintaining documentation standards in accordance with HIPAA regulations.
• Proven autonomy and confidence in managing therapy sessions independently within residential environments.

Compensation and Benefits

St. Elara Medical Center offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience.
