Key Responsibilities\n

• Maintain accurate documentation of client interactions, treatment plans, and progress notes in compliance with agency standards.

• Support clients in their daily living activities, promoting independence by teaching essential life skills such as meal planning and budgeting.

• Monitor and report on clients’ mood and general wellbeing, ensuring adherence to safety protocols within the community health framework.

• Provide medication management assistance, including reminders and ensuring medications are stored and taken properly according to prescribed guidelines.

• Facilitate transportation for clients to appointments and assist them in utilizing public transport efficiently.

• Oversee inventory and availability of essential household supplies, coordinating logistics to meet client needs.

• Collaborate with other healthcare professionals, engaging in regular team meetings to enhance the overall quality of care.

Patient Population / Client Focus\n

Your work will primarily focus on a multicultural and diverse group of clients within the community, addressing various mental health challenges and life transitions. Qualifications\n

• A Master’s degree in Mental Health Counseling, Social Work, Psychology, or a related field is required.

• Current licensure as a LCSW, LMFT, LPCC, or equivalent in California is mandatory. Applications with associate licenses cannot be accepted.
