Key Responsibilities

• Conduct thorough evaluations of patients, which include reviewing medical history, performing physical assessments, and developing personalized treatment plans based on findings.
• Communicate evaluation results and treatment plans effectively to patients and their families, involving them in the goal-setting process.
• Implement treatment protocols with proficiency, adjusting interventions based on patient progress and specific needs, while adapting to the evolving healthcare environment.
• Collaborate with other healthcare providers when necessary to ensure comprehensive patient care and document any changes in patient conditions proactively.
• Manage documentation diligently, ensuring timely and accurate records of patient evaluations, treatment plans, progress notes, and discharge summaries that clearly depict the patient's rehabilitation journey.
• Adhere to productivity benchmarks while managing time effectively, attending meetings punctually, and contributing actively to marketing initiatives within the community.
• Support departmental goals by engaging in professional development and participating in state and federal regulatory compliance efforts.
• Take an active role in mentoring and instructing students and newer staff as part of our commitment to education.

Qualifications
