Key Responsibilities\n

• Conduct thorough assessments to evaluate the emotional and psychological needs of patients and their families dealing with terminal illness.

• Provide individualized and group counseling, utilizing evidence-based interventions tailored to the unique circumstances of each client.

• Develop and implement personalized service plans to support clients in achieving their goals, adjusting these plans as needed based on ongoing assessments.

• Advocate effectively for clients’ rights, ensuring they have access to necessary resources and services while addressing social justice issues they may face.

• Collaborate with healthcare professionals, community organizations, and local agencies to coordinate care and enhance service offerings.

• Participate in ongoing program evaluation, development, and grant reporting to continually improve service delivery.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illnesses and their families, providing the emotional and psychological support they need during a profoundly challenging time. Our clients often come from diverse backgrounds and experiences, requiring a nuanced understanding of each family’s unique circumstances and cultural considerations.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Must possess current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
