Work Environment

Key Responsibilities

• Develop and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscular functions for patients.
• Supervise physical therapy assistants to ensure the effective delivery of treatment protocols and compliance with the established care plans.
• Conduct thorough evaluations of each patient's functional abilities using a combination of diagnostic assessments, including muscle strength and joint mobility tests.
• Assess and recommend modifications to the home environment that are conducive to rehabilitation and independence for patients.
• Deliver prescribed physical therapy services directly in the rehabilitation center or the patient's home, tailoring interventions to meet individual needs.
• Administer a variety of therapeutic exercises, offer guidance on non-manual exercises, and assist patients with functional activities.
• Employ modalities such as traction and massage to enhance recovery and facilitate pain management.
• Evaluate, fit, and adjust prosthetic and orthotic devices as needed, advocating modifications to enhance patient mobility.
• Participate in interdisciplinary team meetings, contributing insights for the development, review, and modification of patient care plans.
• Document patient progress through detailed clinical records, including initial evaluations, ongoing assessments, and discharge summaries, ensuring compliance with OASIS documentation standards.
• Educate patients and their families about the therapeutic devices and overall physical therapy programs.
• Remain accessible to physical therapy assistants during patient treatments to provide guidance and support.
• Adhere to company policies and protocols by submitting clinical documentation and orders in a timely manner.
• Fulfill additional duties as required to assist in achieving the organization's therapeutic and operational objectives.

• Experience in developing and executing comprehensive exercise programs tailored to individual patient needs.
• Familiarity with Electronic Medical Record (EMR) systems, particularly Web PT, for accurate documentation of patient care.

Preferred Experience

• Previous experience in a rehabilitation setting, particularly involving outpatient care.
• Expertise in specialized physical therapy modalities is a plus.
• Strong experience utilizing patient assessment tools for individualized therapy planning.
