Work Environment

This position is based onsite in clients' homes throughout Appomattox, Virginia, which requires traveling to various locations to provide services. With work hours typically set from Monday to Friday, our therapists enjoy a structured schedule that allows for a work-life balance while making significant impacts on patients' lives in their own environments.

Key Responsibilities

• Conduct comprehensive evaluations of clients' physical capabilities, identifying areas for improvement and creating tailored treatment plans that align with each client's goals and physician guidance.
• Direct physical therapy sessions that focus on alleviating pain, preventing disability, and enhancing muscle strength, coordination, and overall functional abilities.
• Supervise and mentor Physical Therapy Assistants to ensure the effective execution of treatment protocols and adherence to established care plans.
• Regularly assess and document patient progress, adjusting treatment plans as necessary based on ongoing evaluations and patient feedback.
• Provide education to clients and their families regarding physical therapy equipment and exercises, fostering a collaborative approach to rehabilitation.
• Participate in interdisciplinary team discussions to coordinate care and enhance treatment strategies, contributing your insights based on clinical evaluations.
• Maintain accurate clinical records, including evaluations, care plans, and discharge summaries, in compliance with OASIS standards and other regulatory requirements.
• Conduct in-home assessments to evaluate the environment and provide recommendations that elevate the safety and independence of clients.

Patient Population / Client Focus

You will care for a diverse group of patients, ranging from post-surgical recovery individuals to those managing chronic illnesses. Your interventions will support their transition back to independence and enhance their quality of life through targeted therapy, education, and support. Understanding the unique challenges faced by each patient in their home environment is crucial for promoting effective rehabilitation.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Minimum of two years of relevant experience in physical therapy, preferably in home care settings.
• Current state licensure to practice Physical Therapy and CPR certification.
• Valid driver's license and automobile insurance.

Compensation and Benefits

We offer a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
