Work Environment

This is a full-time position based onsite in the comfort of patients' homes in Appomattox and surrounding areas. Key Responsibilities

• Conduct comprehensive assessments of patients' physical abilities in their home environment to develop personalized treatment plans.
• Utilize various therapeutic techniques such as exercise prescriptions, manual therapy, and modalities to alleviate pain and improve function.
• Establish, implement, and modify individualized treatment plans, ensuring they align with patient goals and adapt to their evolving needs.
• Document treatment plans, patient progress, and responses diligently to meet best practices and regulatory standards.
• Reassess therapeutic goals regularly and adjust treatment approaches based on patient progress and feedback.
• Provide education and training for patients and caregivers on exercises, the use of assistive devices, and home therapy techniques.
• Stay current with relevant education trends and practices through workshops, professional societies, and peer networking.

Patient Population / Client Focus

Your primary focus will be on home care patients recovering from surgical interventions, injuries, and chronic conditions. You will work closely with individuals across various age groups, tailoring your approach to their unique needs. Our patients often require compassionate support that respects their personal spaces while helping them achieve rehabilitation goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• A valid state licensure to practice physical therapy in Virginia.
• Excellent verbal and written communication skills to effectively engage with patients, caregivers, and healthcare team members.
• Competency in word processing and familiarity with electronic health record (EHR) systems are advantageous.
• Self-motivated, adaptable, and flexible in a dynamic home care environment.
• While two years of clinical experience are preferred, we welcome applicants who are recent graduates and eager to grow.

Preferred Experience

• Experience in home health care or outpatient settings is a plus.

Compensation and Benefits
