Key Responsibilities

• Examine, evaluate, and create individualized treatment plans for patients with various functional limitations and disabilities, ensuring that each plan aligns with patient-specific goals.
• Conduct thorough evaluations, including reviewing medical histories, performing physical assessments, and documenting findings comprehensively.
• Implement treatment protocols based on evaluation outcomes, adapting methods to each patient's evolving condition and needs.
• Collaborate with interdisciplinary healthcare teams to optimize patient care, recognizing when modifications to treatment plans are necessary.
• Ensure that discharge planning is handled efficiently, providing patients with a clear and supportive transition to follow-up care.
• Maintain accurate documentation of patient progress, treatment plans, and related activities to reflect compliance with healthcare regulations and to inform future care decisions.
• Participate in facility meetings and contribute to initiatives aimed at improving patient access and care quality.
• Stay current with professional development, including ongoing training and compliance with state and federal regulations.

Patient Population / Client Focus

In this role, you will be working with a diverse range of patients requiring physical therapy for orthopedic conditions. These may include post-operative rehabilitation, sports injuries, chronic pain management, and geriatric care. Your role will be to empower these patients through tailored treatment plans that aim for optimal recovery outcomes, improving their quality of life.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current state licensure as a Physical Therapist (PT) issued by the Physical Therapy Examining Committee.
• A minimum of two years of outpatient orthopedic experience, enabling you to proficiently assess and treat a variety of conditions.
• Current CPR certification.

Preferred Experience

While two years of outpatient orthopedic experience is required, candidates with additional specialties such as sports rehabilitation or manual therapy techniques will be highly regarded. Experience in home care settings is also beneficial, as it equips you with skills essential for patient engagement in this unique environment.

Compensation and Benefits
