Key Responsibilities\n

• Provide compassionate counseling and crisis intervention, assessing and addressing emotional and behavioral challenges faced by patients.

• Collaborate closely with physicians and healthcare team members to monitor and report on patient progress, needs, and any arising concerns.

• Maintain comprehensive and timely documentation, including clinical and progress notes, in accordance with departmental policies to support continuity of care.

• Engage actively in treatment planning, including creating and reviewing discharge plans based on thorough psychosocial assessments.

• Establish productive working relationships with community resource leaders and actively represent the organization in meetings and committees that address the social needs of our patients.

Patient Population / Client Focus\n

The Licensed Mental Health Counselor will primarily work with hospice patients struggling with a range of mental health issues as they navigate the complexities of end-of-life care. This role requires a nuanced understanding of grief, loss, and the psychological impacts associated with terminal illness. Counselors will be tasked with developing and providing personalized interventions that address both psychological and emotional aspects of care.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Must possess one of the following independent licenses: Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• Minimum of two (2) years of professional experience in a similar setting is necessary.
