Key Responsibilities\n

• Collaborate with each client to develop a comprehensive aftercare plan that prepares them for successful transition post-discharge from treatment.

• Actively communicate with families, external clinicians, and referral sources as necessary to ensure continuity of care and support client recovery.

• Maintain accurate and timely documentation in accordance with company policies, including biopsychosocial assessments and treatment plans.

• Deliver evidence-based therapy, both in individual and group settings, tailored to the unique needs of each assigned client.

• Conduct weekly goal-setting sessions to monitor progress, adapt treatment strategies, and document developments within client records.

Qualifications\n

Compensation and Benefits\n
