Work Environment

This position is based onsite in patients' homes throughout the Highland Park area, allowing you to provide care from the comfort and familiarity of their own environments. Key Responsibilities

As a Physical Therapist with Confluent Rehabilitation, you will:

• Design and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle function as per physician's orders.
• Conduct comprehensive evaluations of each client's functional abilities using established diagnostic and prognostic tests to tailor treatment approaches.
• Supervise and guide Physical Therapy Assistants in delivering therapies while ensuring compliance with the established plan of care.
• Assess the client's home environment, recommending modifications that promote safety and independence during recovery.
• Perform therapeutic exercises, including manual therapy techniques, and instruct clients on exercises that can be completed independently.
• Administer equipment such as prosthetics and orthotics, making necessary adjustments and providing recommendations to enhance patient mobility.
• Meticulously document patient evaluations, progress notes, and treatment plans in compliance with OASIS and clinical recording standards.
• Actively participate in multidisciplinary team meetings to discuss and evaluate patient progress, care strategies, and interdisciplinary collaboration.
• Instruct clients and their families on the effective use of physical therapy devices and overall treatment plans.
• Maintain timely and organized records for clinical notes, therapy sessions, and administrative requirements to ensure smooth operational processes.

Patient Population / Client Focus

You will work with a diverse patient population ranging from post-operative recovery individuals to those managing chronic illnesses, all within the comfort of their homes. Qualifications

The successful candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid DPT license with current standing in the state of New Jersey.
• At least two years of hands-on experience in physical therapy, preferably within a home care environment.
• Certification in CPR and a valid driver's license with reliable transportation.
• Strong leadership skills, capable of making decisions that enhance patient care outcomes.
• Exceptional communication skills to engage effectively with patients, families, and team members.

Preferred Experience

While a minimum of two years of clinical experience is essential, candidates with experience in home health care or an acute care setting will be highly regarded. A solid understanding of rehabilitation practices, regulatory requirements, and node-level management with patients is a plus. Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 per year, along with a comprehensive benefits package that includes healthcare, dental, vision, and a Flexible Spending Account.
