Key Responsibilities\n

As a Licensed Professional Counselor, you will:

• Assist primary care providers in identifying and managing mental health and psychosocial issues, fostering a collaborative team approach.

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, while offering recommendations to primary care providers regarding treatment goals.

• Provide brief, focused interventions for patients requiring mental health services, monitoring their progress and adjusting treatment plans accordingly.

• Advise on the need for referrals to specialized mental health facilities and implement follow-up procedures to assess patient wellbeing.

• Develop relapse prevention plans and ensure delivery of coordinated health services, integrating behavioral care into overall treatment.

• Document patient progress, maintaining accurate clinical records and comprehensive reports to support treatment continuity.

• Educate patients on self-management strategies, empowering them to actively participate in their treatment and recovery processes.

• Collaborate with clinic management and other team members to evaluate and enhance behavioral services.
