Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, followed by recommendations for treatment goals.

• Work alongside primary care providers to identify patients best suited for continued care within the primary care environment versus those needing specialized mental health services.

• Follow up with patients post-intervention to assess progress and make necessary adjustments to treatment plans.

• Develop and implement relapse prevention strategies to help patients maintain stability.

• Assist in the early detection of at-risk patients and formulate proactive plans to prevent deterioration of their mental health.

• Document patient progress meticulously and communicate updates to primary care providers, ensuring a unified approach to healthcare.

• Provide ongoing education to patients aimed at self-management and making informed lifestyle choices that enhance their overall health.

• Collaborate with clinic management to evaluate program effectiveness and suggest enhancements to behavioral health services.

Patient Population / Client Focus\n

This role primarily focuses on a diverse patient demographic, ranging from children to adults with a variety of mental health needs. The counselor will provide tailored support designed to address complex psychosocial issues influenced by cultural and community factors unique to Columbia, TN. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
