Work Environment\n

This role provides the flexibility of a remote work environment, catering to your lifestyle while allowing you to focus on delivering high-quality care. You will work as part of a collaborative team in a community health clinic setting, engaging with both medical and mental health professionals to enhance patient outcomes. Your primary work hours are Monday through Friday, 8:00 AM to 5:00 PM, with occasional evening shifts as needed by patient demand.

Key Responsibilities\n

Patient Population / Client Focus\n

Your work will primarily focus on patients who experience complex needs due to co-occurring mental health and medical conditions. You will be instrumental in providing care for individuals managing chronic health issues such as diabetes, hypertension, or cancer, alongside their mental health diagnoses. Understanding the psychological impact of physical health issues is crucial in this role.

Qualifications\n

• A master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure in Georgia as an LCSW, LPC, or LMFT.

• Strong knowledge and experience in addressing mental health challenges related to chronic medical conditions.

• Proficient in providing integrated care and working collaboratively within a clinical team.

• Excellent communication and interpersonal skills, enabling rapport with patients and team members.

Preferred Experience\n
